MHM creates opportunities for Minnesotans to move from institutions to their own home in the community.
This medical assistant benefit allows the advocacy and support needed to find stable housing for people with disabilities and seniors facing homelessness.
Services that assist a person to plan for, find, and move to a home in the community. With this service we will assist with the housing search, and application process.
Services that support retaining a new home.
A service available under the Moving Home Minnesota program that offers a variety of comprehensive community support.
